New Patient?

If you are a new patient to the clinic, please book a longer (20-minute) appointment for your first visit. Please remember to book an appointment for each person wishing to be seen.

If you are booking online for the first time, you will need to provide registration details, including your current Medicare card details. Please ensure the clinic is aware of any changes to your card, as our online booking system will only work if the clinic has your latest information.

When registering/booking on-line, please ensure you use the details as they appear on your Medicare Card.

What to bring to Your first appointment?

Here is a list of items you should always bring to your appointment

  • Medicare or concession card(s) if applicable
  • Medication List – including prescription or non-prescription medicines and any herbal therapies Paperwork, if applicable
  • Sometimes it is hard to remember your questions. It may help to write them down and bring them with you to your appointment.

Opening Hours

The surgery opens at 8.30am Monday to Friday to approximately 5.30pm most days.

We are also open on Saturday from 8.30am to 11.30am.

Closed on Sundays and Public Holidays

Out Of Surgery Visits

If you are too sick to attend the surgery, house calls can be arranged: however,
Doctors will only make house calls where urgency and circumstances dictate.

Our doctors pride themselves on running on time. Should you attend for a
more complex issue or two issues please book a long consultation.

Appointments

Appointments can be made by ringing 9454 5544 between 8.15am – 5.30pm weekdays, and between 8.15am and 11am on Saturdays. Every effort will be made to accommodate your preferred Doctor and time. Emergencies are always given priority and may at times cause delays. Please advise our reception staff if you require a longer appointment and always book an appointment for each person requiring consultation. 

Appointments can also be made online through Hotdoc.

Interpreter Services are available on 13 14  50 if needed. Please advise our staff at the time of booking your appointment if this is required.

Missed appointments Fee/Late Cancellation

4 hours notice of cancellation must be given or a fee of $44 may be charged. Non attendance of a booked appointment may incur a fee of $44. Please note that patients not attending appointments that they have booked through Hotdoc will be charged a non-attendance fee of $44 as Hotdoc confirms your booking when it is made.

Telephoning Your Doctor

The Doctors will take calls relating to you and your family’s health. lf they are in consultation, the receptionist will take a message and he/she will return your call. Please advise staff of the urgency of your call.

Results

Your Doctor will advise you if you are to make a follow up appointment for your results.

Reminder System

Our practice is committed to preventative care. Your Doctor will seek your permission to include you on our reminder system which, together with the National and State registers and reminder systems, will enable us to offer you preventative health services appropriate to your care. If you do not wish to be part of this system please let your Doctor or our reception staff know.

Repeat Prescriptions

All patients are requested to attend for repeat prescriptions. Please tell our staff that your appointment is for a prescription only and we will fit you in with a Doctor. If you are attending for more than a prescription, a normal appointment will need to be made.

Referrals

All patients are requested to attend for referral. If you see a specialist on an on-going basis, please check with them if your referral is current. We cannot backdate referrals, so it is important to obtain a current one prior to your specialist appointment.

Electronic Communication

Please note that this Practice DOES NOT accept any Email requests to our Doctors.

Transfer of Medical Records

This Practice charges a fee of $30 + $10 for each additional family member to transfer records to another Practice. Once payment has been received, we will forward your records to the requesting Doctor/Surgery.

Specialists Fees

Should you be referred to a Specialist by one of our Doctors, please be advised that specialist’s fees vary and it is advisable to enquire about their charges when booking your appointment with them.

Management of Personal Health information

Your medical record is a confidential document. lt is the policy of this practice to maintain security of your personal health information at all times and to ensure that this information is only available to authorised personnel. To this end this practice has developed a privacy policy that complies with the privacy legislation and the NPPS.

We take your concerns, suggestions and complaints seriously, therefore if you have a problem, we would like to hear about it. Please feel free to talk to your Doctor or our Practice Manager, Office Manager or you may prefer to write to us.

After Hours Care

If medical care is required outside of our practice hours, we recommend our patients access Dial-a-Doctor. Doctor Home Visits is a medical service provided by Dial-a-Doctor and exists solely to offer patients home medical care both in-hours and after-hours on behalf of their own GP.

A fee may be charged for this service.

For Emergencies dial 000, or attend St John of God Murdoch (Private), St John of God Midland or Royal Perth Hospital Emergency Departments. After-hours GP services are available from GP After Hours, Beaufort Street, Mount Lawley 7pm – 11pm weekdays and 2pm -10pm Sat and 10am – 10pm Sun – Tel: 9370 4200
